If you would like access to your medical records or someone else’s medical records, please read the leaflet below and then complete the relevant application form(s). The form(s) then need to be returned to the surgery. The leaflet explains the possible costs and the timescales the surgery will work to. The leaflet also explains whose records you can request to access.
